Hypnosis is alternative or complementary health care under the laws of The American Board of Hypnosis (Internationally recognised), covered under the Insurance Policy of AON, ABN 74540043519. I am not licensed that means that I am not a Medical Doctor, Psychiatrist, Psychologist, Social Worker or a Counsellor and you acknowledge that you have never worked with a licensed person on this problem (if you have, you need to present me a letter of referral from your Medical Practitioner before we start therapy or coaching).
If you have been prescribed medication relating to your clinical Hypnotherapy Session prescribed by your Medical Practitioner, you need to present me a letter of consent from him/ her before we start therapy.
This means a short written note stating that you have informed your health care provider of your decision to engage in Hypnosis. Furthermore, nothing that happens here in the session should be construed as, nor should you believe that it is a substitute for the advice of a licensed person.
Privacy
In accordance to the TAD James Policy Act, I keep all information that I receive from you, confidential, and do not disclose it to persons outside my clinic. All client information is kept strictly confidential, is for internal use only and cannot be disclosed without your consent. Importantly, I am required to report and share information about child abuse with state bodies who oversee such matters.
Cancellation Policy
The appointment that you have made is reserve entirely for you. Because of this, I require 24 hours notice for cancellation of a Clinical Hypnotherapy Session. Please adhere to the notice period as it allows sufficient time to offer the appointment to someone else.
